These Terms & Conditions govern your access to and use of LON6EVITY, including lon6evity.com, the LON6EVITY web application, subscription plans, free trials, premium digital features, longevity profile, biomarker tracking, prevention tools, doctor-ready reports, medical document storage, wearable-device readiness features, Executive / Medical Review workflow and the World Emergency Card emergency profile connection.

Please read these Terms carefully before using LON6EVITY.

1. Provider

In these Terms, “LON6EVITY”, “we”, “us” or “our” means MAŃSKI s.r.o.

“You” or “user” means any person accessing or using LON6EVITY.

2. Acceptance of these Terms

By accessing LON6EVITY, creating an account, starting a free trial, purchasing a subscription, applying for Executive / Medical Review or using any platform feature, you agree to these Terms.

If you do not agree, you must not use LON6EVITY.

If you use LON6EVITY on behalf of another person, company or organisation, you confirm that you have authority to do so.

3. What LON6EVITY provides

LON6EVITY is a premium digital health-organisation and longevity-intelligence platform. Depending on your plan, it may provide:

• Prevention Calendar;
• Biomarker Tracking;
• Advanced Biomarker Dashboard;
• Biomarker Trend Analysis;

LON6EVITY helps users organise, structure and understand health-related information. It does not replace a doctor, hospital, emergency service, medical examination, laboratory, licensed medical consultation or clinical diagnosis.

4. Important medical disclaimer

LON6EVITY is not a medical device, medical service, healthcare provider, diagnostic tool, treatment provider, emergency service or substitute for professional medical advice.

Any scores, dashboards, reports, biomarker interpretations, risk-awareness outputs, prevention suggestions, questions for your doctor or Executive / Medical Review outputs are provided for informational, organisational and educational purposes only.

You must always consult a licensed healthcare professional before making decisions about diagnosis, treatment, medication, supplements, medical testing, lifestyle changes, exercise, pregnancy, chronic disease management or emergency care.

Never ignore professional medical advice or delay seeking medical help because of information displayed by LON6EVITY.

If you believe you may have a medical emergency, contact local emergency services immediately.

5. Eligibility

You must be at least 18 years old to create a LON6EVITY account and purchase a subscription.

If you use LON6EVITY for another person, such as a family member, employee or dependent, you confirm that you have the legal right and necessary permission to enter and manage that person’s data.

6. Account registration

To use paid features, you may need to create an account.

You agree to:

• provide accurate and current information;
• keep your login credentials secure;
• not share your account with unauthorised persons;
• notify us of any suspected unauthorised access;
• keep your health information, emergency contacts and documents updated.

You are responsible for all activity under your account unless caused by our breach of these Terms or applicable law.

7. Subscription plans

LON6EVITY offers premium, annual-only digital subscriptions.

7.1 LON6EVITY Plus

Price: $249 per year
Trial: 7-day free trial, where available

Designed for users who want a premium prevention profile, Longevity Readiness Score, prevention calendar, biomarker tracking and World Emergency Card emergency profile.

Main features may include:

• Longevity Readiness Score;
• Missing Health Data;
• Prevention Calendar;
• Biomarker Tracking;
• Emergency Profile by World Emergency Card;
• Doctor-ready PDF.

7.2 LON6EVITY Premium

Price: $399 per year
Trial: 7-day free trial, where available

Designed for users who want deeper longevity analytics, advanced biomarker trends, wearable sync readiness, doctor-ready reports, medical document storage and advanced prevention insights.

Main features may include:

• everything in Plus;
• Advanced Biomarker Dashboard;
• Biomarker Trend Analysis;
• wearable-device sync readiness;
• Advanced Prevention Calendar;
• Risk Awareness Dashboard;
• Medical Document Storage;
• Annual Longevity Report.

7.3 LON6EVITY Executive / Medical Review

Price: from $999 per year
Trial: no automatic free trial unless expressly stated in writing

Designed for executives, founders, high-performance professionals and premium clients who want advanced health intelligence, medical document review workflow, executive reporting and personal prevention priority mapping.

Main features may include:

• everything in Premium;
• Executive Health Intelligence Report;
• Medical Document Review workflow;
• Personal Prevention Priority Map;
• Questions to Ask Your Doctor;
• Expert Review workflow;
• Premium Support;
• Custom onboarding.

Executive / Medical Review may require application, eligibility review, onboarding, additional information, availability of expert workflow capacity and separate confirmation.

8. Free trials

LON6EVITY Plus and LON6EVITY Premium may include a 7-day free trial.

By starting a free trial, you agree that:

• you receive access to selected paid features during the trial period;
• unless you cancel before the end of the trial, your annual subscription may begin automatically;
• the payment method provided may be charged for the applicable annual price after the trial ends;
• the trial is available only once per user unless we expressly allow otherwise;
• we may withdraw or modify trial availability for future users.

You can cancel your trial anytime before it converts into a paid subscription.

9. Annual subscriptions and renewal

LON6EVITY subscriptions are annual-only.

Unless cancelled before renewal, your subscription may renew automatically for another annual period, depending on the checkout flow, payment provider settings and applicable law.

We will present key payment terms before purchase, including plan, price, billing period and renewal information.

You are responsible for ensuring that your billing information is accurate and current.

10. Cancellation

You may cancel your subscription anytime through your account settings, payment portal or by contacting hello@lon6evity.com.

Cancellation stops future renewal. Unless required by applicable law or expressly stated otherwise, cancellation does not automatically entitle you to a refund for the current paid annual period.

After cancellation, you may continue to access paid features until the end of the paid subscription period, unless the subscription is terminated for breach, fraud, misuse or legal reasons.

11. Refunds and consumer withdrawal rights

11.1 General refund policy

Because LON6EVITY is a digital service with immediate access to premium features, annual subscription fees are generally non-refundable after payment, except where required by applicable law or expressly approved by us.

11.2 Free trial protection

If your plan includes a 7-day free trial and you cancel before the trial ends, you will not be charged for the annual subscription.

11.3 EU consumer withdrawal rights

If you are a consumer in the European Union, you may have a statutory right to withdraw from an online contract within 14 days, subject to legal exceptions and rules for digital content and digital services.

Where you request immediate access to digital services or digital content during the withdrawal period, you may be asked to provide express consent to immediate performance and acknowledge the consequences for your withdrawal rights under applicable law.

Nothing in these Terms limits any mandatory consumer rights that cannot legally be excluded.

To exercise a withdrawal right, contact us at hello@lon6evity.com with a clear statement that you withdraw from the contract. You may use the model withdrawal form below, but it is not mandatory.

12. Prices, taxes and currency

Prices are displayed in USD unless stated otherwise.

The final amount may include applicable taxes, VAT or other charges depending on your location, billing details and applicable law.

Currency conversion fees or bank charges may be applied by your bank or payment provider. We are not responsible for such third-party fees.

13. Payment processing

Payments are processed by third-party payment providers. By purchasing a subscription, you agree to the applicable payment provider’s terms and processing of payment data.

We may suspend or restrict access if payment fails, is reversed, disputed, fraudulent or otherwise not completed.

14. Executive / Medical Review

Executive / Medical Review is a premium workflow designed to help organise, review and present health-related information in a structured way.

Unless expressly confirmed in writing, Executive / Medical Review does not mean:

• diagnosis;
• treatment;
• emergency care;
• prescription;
• laboratory testing;
• doctor-patient relationship;
• guaranteed review by a specific physician;
• guaranteed health outcome.

Any expert or medical review workflow is limited by the data you provide and by the availability, qualifications and scope of participating professionals.

You remain responsible for consulting your own licensed healthcare providers.

15. User data and responsibility for accuracy

LON6EVITY depends on information provided, uploaded or connected by you.

You are responsible for ensuring that:

• your data is accurate;
• your medication list is current;
• allergies and emergency information are updated;
• biomarker values are entered correctly;
• uploaded documents are complete and relevant;
• information about other people is provided lawfully;
• emergency contacts have been informed where required;
• you do not upload illegal, misleading or harmful content.

We are not responsible for harm caused by inaccurate, outdated, incomplete or misleading user-provided information.

16. World Emergency Card emergency profile

Some LON6EVITY plans include or connect to an emergency profile by World Emergency Card.

This feature is intended to make selected emergency information more accessible when needed. It does not guarantee that emergency responders, hospitals, doctors or third parties will access, understand or rely on the information.

You are responsible for keeping emergency information accurate and appropriate for emergency use.

17. Medical document storage

If your plan includes medical document storage, you may upload documents such as lab results, medical reports, imaging summaries, doctor letters or other health-related files.

You must not upload content that:

• violates another person’s privacy;
• contains data you are not authorised to process;
• is unlawful;
• contains malware;
• infringes intellectual property rights;
• is intentionally false or misleading.

We may remove or restrict content where required for security, legal compliance, fraud prevention or platform integrity.

18. Wearable-device sync readiness

LON6EVITY Premium and higher plans may include wearable-device sync readiness or future integration capability.

Wearable-device data may be incomplete, inaccurate, delayed or unavailable depending on the device, third-party provider, user settings and technical compatibility.

LON6EVITY does not guarantee compatibility with any specific wearable device or third-party health platform unless expressly stated.

Wearable data should not be used as the sole basis for medical decisions.

19. Platform availability

We aim to provide a reliable and secure digital service, but we do not guarantee uninterrupted availability.

The platform may be unavailable due to:

• maintenance;
• updates;
• technical failures;
• hosting outages;
• cyber incidents;
• third-party service interruptions;
• payment provider issues;
• force majeure events;
• legal or security reasons.

We may modify, improve, suspend or discontinue features where necessary. We will try to provide reasonable notice for material changes affecting paid subscriptions.

20. Acceptable use

You must not:

• misuse the platform;
• attempt unauthorised access;
• interfere with security features;
• scrape, copy or reverse-engineer the platform;
• upload malware;
• submit fraudulent data;
• impersonate another person;
• use the service for illegal purposes;
• abuse trial offers;
• resell access without permission;
• use LON6EVITY to provide unauthorised medical services;
• use outputs as a substitute for licensed medical advice.

We may suspend or terminate accounts that violate these Terms.

21. Intellectual property

LON6EVITY, including its design, software, text, structure, dashboards, reports, scoring logic, branding, trademarks and platform content, is owned by or licensed to MAŃSKI s.r.o.

You receive a limited, personal, non-exclusive, non-transferable, revocable right to use the platform according to your subscription plan.

You may use doctor-ready PDFs and reports generated for you for personal health organisation and communication with your healthcare professionals. You may not copy, resell, reproduce or commercially exploit the platform or its content without our written permission.

22. Your content

You retain rights to personal data, documents and information you upload.

You grant us the right to process, store, display and technically use your content as necessary to provide LON6EVITY features, comply with law, secure the platform and fulfil these Terms.

You confirm that you have all rights and permissions required to upload and process the content you provide.

23. Privacy and data protection

Our processing of personal data is described in the LON6EVITY Privacy Policy.

By using LON6EVITY, you acknowledge that health-related data may be processed to provide the service, subject to your explicit consent where required by law.

24. Third-party services

LON6EVITY may rely on third-party services, including hosting, payment processing, analytics, security, email delivery, document storage, wearable-device providers or World Emergency Card infrastructure.

We are not responsible for third-party services that we do not control. Their own terms and privacy policies may apply.

25. No guarantee of health outcome

LON6EVITY does not guarantee:

• longer life;
• prevention of disease;
• early detection of disease;
• improved medical outcome;
• accurate diagnosis;
• emergency response;
• doctor acceptance of reports;
• specific biomarker improvement;
• specific performance improvement;
• compatibility with all medical systems or devices.

The platform is a tool for organisation, awareness and preparation. Health decisions must be made with qualified healthcare professionals.

26. Limitation of liability

To the maximum extent permitted by applicable law, MAŃSKI s.r.o. is not liable for indirect, incidental, special, consequential or punitive damages, loss of data, loss of profit, loss of business, health outcomes, medical decisions or reliance on platform outputs.

Nothing in these Terms excludes liability that cannot be excluded under applicable law, including liability for intentional misconduct, gross negligence, personal injury caused by legally proven fault, or mandatory consumer rights.

For paid users, and to the maximum extent permitted by law, our aggregate liability relating to the service is limited to the amount you paid for LON6EVITY during the 12 months preceding the event giving rise to the claim.

27. Suspension and termination

We may suspend or terminate your access if:

• you breach these Terms;
• payment fails;
• we detect fraud, misuse or security risk;
• required by law;
• your use creates risk to us, users or third parties;
• you misuse health, emergency or document features;
• you abuse free trials or payment systems.

You may stop using the service and cancel your subscription at any time.

Termination does not affect rights or obligations that by their nature should survive, including payment obligations, intellectual property, disclaimers, liability limits, governing law and data retention obligations.

28. Changes to the service

We may update, improve, modify or remove features to reflect:

• technical development;
• security requirements;
• legal requirements;
• medical-safety considerations;
• product strategy;
• user experience improvements;
• third-party service availability.

If a material change significantly reduces the core paid functionality of your plan, we will take reasonable steps to inform you.

29. Changes to these Terms

We may update these Terms from time to time.

If changes are material, we will provide notice through the website, application or email where appropriate.

Continued use of LON6EVITY after the effective date of updated Terms means you accept the updated Terms, unless applicable law requires a different process.

31. Alternative dispute resolution

If you are a consumer, you may have the right to use an alternative dispute resolution body depending on your country and the nature of the dispute.

For consumers in the Czech Republic, the Czech Trade Inspection Authority may be competent for certain consumer disputes unless another authority has jurisdiction.

Nothing in these Terms limits your mandatory consumer rights.

32. Governing law and jurisdiction

These Terms are governed by the laws of the Czech Republic.

If you are a consumer residing in the European Union or another jurisdiction with mandatory consumer protection rules, you may also benefit from mandatory protections of your country of residence.

Any disputes shall be resolved by the competent courts of the Czech Republic, unless mandatory consumer law gives you the right to bring proceedings elsewhere.

33. Severability

If any provision of these Terms is found invalid, unlawful or unenforceable, the remaining provisions remain valid and enforceable.

The invalid provision will be replaced or interpreted in a way that most closely reflects its original purpose while complying with applicable law.

34. No waiver

If we do not enforce a provision immediately, this does not mean we waive our right to enforce it later.

35. Entire agreement

These Terms, together with the Privacy Policy and any plan-specific checkout terms, form the agreement between you and MAŃSKI s.r.o. regarding LON6EVITY.
